Incoming 6th Grade Student Info

All students entering 6th grade must show proof of one meningococcal conjugate vaccination (MCV) given on or after 11 years of age. All student health records (physical, immunization, dental or vision forms) should be emailed directly to medicalrecords@hawthorn73.org.

Tetanus/Diptheria/Acellular Pertussis (Tdap) Vaccination

Any child entering 6th, 7th or 8th grade must show proof of receiving one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose. As further reminder, children entering 6th grade need to have a completed hepatitis B series, a full physical (Illinois Child Health Exam form) and dental exam (Illinois Dental Exam form) and parents must also provide Proof of Residency.
